Three ideas they will implement. (Tells me what they found most important.) 2. A one- or two-sentence reaction to program and speaker. I ask for name and position if they give me permission to quote them. (Now I have a source for promo quotes.) 3. Name, phone, etc. of someone to whom they would recommend the program or speaker. (Now I have leads.) 4. Option to sign on for my electronic newsletter. (Now I have leads for product sales, free promo opportunities, etc.) I also have the name of the program, date presented and group to whom I'm speaking in the heading. I used to ask for numerical ratings on all sorts of things. This is much more meaningful, gets the job done, works great and provides lots of information.
